Position Purpose The Intervention Specialist (on-site) will be
responsible for providing immediate assistance to individuals with
urgent needs enrolled in or seeking services through Rocky Mountain
Human Services. The Intervention Specialist is responsible for
providing frontline engagement and assistance as required. In
crisis situations the Intervention Specialist will interface with
and provide immediate solutions for those seeking assistance. For
less urgent situations, the Intervention Specialist will
communicate with the case manager and provide follow-up until it is
clear the need is addressed. This role will involve consulting with
case managers to ensure that community resources and alternative
approaches are available and utilized effectively and serving as a
liaison for clients being supported within the Case Management
Agency and Developmental and Behavioral Health departments (serving
people who have intellectual and developmental disabilities).This
position will focus on working with people who live in Denver City
and County and connecting people to resources in the Denver metro
area. The Intervention Specialist must be familiar with all RMHS
programs and be present in the office to assist walk-ins
effectively. Essential Duties Provides frontline, accessible and
immediate support to individuals enrolled in or seeking services
through Rocky Mountain Human Services. Serves as a resource for
RMHS case managers, acting in their capacity when needed and
appropriate. Builds relationships with community resources and
maintains a list of resources to support with stabilization efforts
Assesses needs and directs resources to program areas as needed. Ex
ensuring Mill Levy resources are utilized for all eligible people.
Supports, assists, and advocates for people when community
resources are needed. Builds relationships with community resources
and/or partners and maintains a list of resources to help with
emergency or urgent situations. Establishes effective working
relationships with major hospitals that support individuals
enrolled in or seeking RMHS services. Works with the Regional
Accountable Entity (RAE) to get individuals connected to behavior
services or medical services. Works with county-based Adult
Protective Services to refer people when needed or to support the
case manager. Develops and coordinates housing search training for
clients and case managers Produce timely data reports, including
outcome measures, quarterly and annual reports for city contract
Works closely with Mill Levy team to access funding for necessary
support when appropriate. Actively facilitates communication among
individuals enrolled in or seeking services, caregivers, families,
stakeholders, advocates and providers. Attends staff, team and
department meetings. Attends in-services, staffing and other
meetings with supervisor's approval. May be appointed to
committees. Participates in agency and community planning and
education. Develops and maintains client case records, plans and
reports. Maintains prompt and regular attendance. Performs related
work as assigned. Knowledge, Skills, and Abilities Knowledge and
experience with case management, specifically with HCBS waivers and
all programs offered through RMHS. Ability to provide support in
various settings, based out of the main office. Understands
systematic services and supports and how to access services outside
of waiver or state funded programs. Experience with intervention
management and getting people connected to supports. Strong
problem-solving skills Stays organized and can be flexible during
Intervention to get people connected to services. Collaborates with
case managers and teams to ensure people receive the resources
needed. Takes initiative and has a willingness to pick up needed
tasks in order to get people connected to services. Ability to
create and compose written materials Ability to make reasonable and
sound evaluative judgements Ability to effectively and efficiently
respond to questions from persons served Comfortable building
relationships face to face Builds rapport with a variety of
internal and external stakeholders. Understanding of RMHS's mission
and values and ability to represent the agency when building
community relationships. Strong advocate for people to get the
services and supports they need. Ability to act independently and
make decisions within scope of the position. Essential Functions

drive personal or company vehicle Minimum Qualifications Bachelor's
degree in human services or a related field Two years' experience
providing case management services Experience working with
community resources and assisting people through Intervention
situations Preferred Qualifications Master's degree in human
services, social work, or related field Employment Type: Full Time
